For the application, please use a font size of no smaller than Arial 11 in the main document with half inch margins, and tables with font size of no smaller than Arial 9.

All documents must be compiled into a single PDF and submitted by the deadline through the SPARTAN website. All applications should include the following components:

• Description of the rheumatology fellowship training program, with details on SpA focus (up to 2 pages)
• NIH biosketch of the training program director (up to 5 pages)
• CV or NIH biosketch of the fellow (up to 5 pages)
• Letter of institutional support (1 page): The division chief, department chair, or equivalent institutional official must provide written assurance of the following:
  • quality of the training program, including focus on SpA
  • clear support for the program, Program Director and trainees
  • means of funding remaining salary (how the remaining funding for the trainee will be covered in the award cycle)

For Clinical track applicants, please also include the following:

• Individualized training plan, with details on SpA focus (1 page)
• Letter of support (1 page) and NIH biosketch of the clinical mentor (up to 5 pages). This documentation must specify that the mentor is a current SPARTAN member.

For Research track applicants, please also include the following:

• Outline of research project, with details on SpA focus. Please include a timeline for project completion (1 page)
• Letter of support (1 page) and NIH biosketch of the research mentor (up to 5 pages). A letter of support from a research mentor who is a SPARTAN member is required. This documentation must specify that the mentor is a current SPARTAN member.
